Effect of ulinastatin combined with BiPAP noninvasive positive pressure ventilation on patients with AECOPD complicated with type Ⅱ respiratory failure

Abstract：Objective: To analyze the clinical effect of ulinastatin combined with BiPAP noninvasive positive pressure ventilation on acute exacerbation of chronic obstructive pulmonary disease(AECOPD)complicated with type Ⅱ respiratory failure.Methods: The clinical data of 73 patients with AECOPD combined with type Ⅱ respiratory failure from Nov 2017 to Dec 2019 were retrospectively analyzed.And 34 patients treated with BiPAP noninvasive positive pressure ventilation were selected as control group,39 patients treated with ulinastatin on the basis of the control group were selected as the study group.The curative effect and pulmonary function[percentage of forced vital capacity in expected value(FVC%),percentage of forced expiratory volume in one second in expected value(FEV1%)],arterial blood gas indexes[arterial partial pressure of oxygen(PaO2),arterial partial pressure of carbon dioxide(PaCO2)],serum levels of inflammatory factors[interleukin 6(IL-6),leukotriene B4(LTB4)]before and after treatment were compared between the two groups.Results: The total effective rate of the study group was 89.74%(35/39),which was higher than 70.59%(24/34)of the control group(P<0.05).After treatment,FVC% and FEV1% in the study group were higher than those in the control group(P<0.05).After treatment,PaO2 in the study group was higher than that in the control group,and PaCO2 was lower(P<0.05).After treatment,the serum levels of IL-6 and LTB4 in the study group were lower than those in the control group(P<0.05).Conclusion: Ulinastatin combined with BiPAP noninvasive positive pressure ventilation in the treatment of AECOPD combined with type Ⅱ respiratory failure has good effect,can reduce inflammatory reaction,and improve pulmonary function and arterial blood gas index.
